Effective April 11, 2026 – We’re excited to announce that we will transition to Epic as our new, unified electronic medical record system.  This is great news for all!  This incredible milestone is a significant step forward in strengthening how we connect care across our network of health centers, hospital partners throughout Chicagoland and to you.

Epic brings patient information together into one secure, integrated platform, streamlining scheduling, documentation, lab results, billing, and even telehealth services. Most importantly, it provides our care teams with a more comprehensive view of your health, enhancing coordination across our family practice, OB/GYN, behavioral health, dental health, wellness and TodayCare (Walk-in) services.

As part of your transition, you will now have access to MyChart, it will be your only patient portal going forward. MyChart, which is an advanced patient portal that makes it easier to manage your care. Through MyChart, you can view your test results and medical records, communicate directly with your care team, schedule appointments, request prescription refills, and access telehealth services all in one convenient place!  It’s as easy as 1.2.3.  You can also merge your existing MyChart accounts together, making it a one-stop-shop for all of your healthcare needs.
